Yes, headache is a listed side effect of GLP-1 medicines, but the label figures show it is only modestly more common than with placebo. In the Wegovy trials, 14% of adults on the 2.4 mg injection reported headache versus 10% on placebo. Medicine is not the only possible cause. Skipped meals, too little sleep, alcohol and dehydration are common headache triggers, and all of them are easy to run into when you are eating and drinking less. What the labels and medication guide say
MedlinePlus lists headache among the common side effects of semaglutide injection. The Wegovy prescribing information reports headache in 14% of adults on the 2.4 mg injection versus 10% on placebo, in three placebo-controlled trials of up to 68 weeks. The adverse reaction table in the Zepbound label does not list headache, so no tirzepatide figure is given here, and nothing in this article says one product causes more headaches than another.
The gap between 14% and 10% is real but small. That is a reminder that headaches are common in everyone, and that a headache on a GLP-1 is not automatically caused by the medicine.
Possible contributors you can notice
An article cannot tell you what is behind your headache. Your prescriber can look at the whole picture. These are triggers that general health sources describe:
- Skipped meals. MedlinePlus says tension headaches are more likely if you work too much, do not get enough sleep, miss meals or use alcohol. GLP-1 medicines reduce appetite, so it is easy to go long stretches without eating.
- Dehydration. MedlinePlus advises people with migraine to drink water to avoid getting dehydrated, especially after vomiting. The Wegovy medication guide notes that diarrhea, nausea and vomiting may cause a loss of fluids (dehydration). If you have those symptoms, see GLP-1 nausea and GLP-1 diarrhea.
- Low blood sugar. The Wegovy medication guide lists headache among the symptoms of low blood sugar, along with dizziness or light-headedness, sweating, shakiness, blurred vision, slurred speech, weakness, anxiety and hunger. Ask your prescriber whether low blood sugar is a concern for you, particularly if you take other diabetes medicines.
- Poor sleep and alcohol. Both are on the MedlinePlus trigger list above. The 2025 joint advisory on nutrition for GLP-1 therapy also suggests going easy on alcohol while on treatment.
Habits people try
These are general habits, not GLP-1 specific treatments.
- Keep fluids steady through the day. Small sips often work better than large glasses when you feel unwell. See GLP-1 drinks and hydration.

- Protect your sleep and limit alcohol.
- Rest in a quiet, dark room and place a cool cloth on your head, which MedlinePlus lists as self-care for migraine.
- Keep a headache diary. MedlinePlus suggests noting timing, diet, sleep and activities to spot triggers.
About pain relievers
Many people reach for over-the-counter pain relievers. This page does not recommend any and does not give doses. Whether one is right for you depends on your health and your other medicines, and MedlinePlus notes that overusing pain medicine can itself cause rebound headaches. Ask your pharmacist or prescriber.
When to call a clinician or get care
MedlinePlus says to get medical help right away for a headache after a blow to the head, or a headache with a stiff neck, fever, confusion, loss of consciousness, or pain in the eye or ear. It says to let your provider know about sudden, severe headaches. Call 911 or local emergency services for a sudden, explosive headache or for headaches with neurological symptoms. In addition, from the GLP-1 labels and medication guide:
- Changes in vision. The Wegovy medication guide says to tell your healthcare provider about changes in vision during treatment.
- Signs of dehydration or kidney trouble: extreme thirst, dark urine, urinating less than usual, dizziness, or swelling of the legs, ankles or feet (NIDDK and MedlinePlus).
- Severe stomach pain that will not go away, with or without vomiting, which may spread to the back. The labels warn about pancreatitis.
- A serious allergic reaction: swelling of the face, lips, tongue or throat, severe rash or itching, or trouble breathing. Get medical help right away.
- A headache that is new, keeps coming back, or bothers you. The Wegovy medication guide says to talk to your provider about any side effect that bothers you or does not go away.
